Home Care Packages

Hume City Council has a long and proud history of connecting with its residents to support them to remain living independently in their own homes for as long as possible.

We believe all people should be valued and respected, connected to their communities and in control of their lives. Your Care Advisor works closely with you to coordinate an individual package of care that will support your lifestyle at home.

Eligibility

You may be eligible for a home care package if you are:

  • an older person who needs coordinated services to help you to stay in your home
  • a younger person with a disability, dementia or other special care needs that are not met through other specialist services.

There are no minimum age requirements or residency restrictions for Home Care Packages. However, the packages are not intended for visitors to Australia or people requiring temporary or short-term care.

Services and support available with Home Care Packages:

Home Care Packages are flexible and offer a broad range of services. Depending on your needs, services can include:

  • Domestic assistance: cleaning and laundry
  • Personal care: assistance with bathing, showering, grooming and dressing
  • Respite care: gives carer's a well earned break 
  • Transport: to assist you to access your community, complete shopping, attend health care appointments and social activities
  • Planned activity groups and community programs: including day programs and outings
  • Home Maintenance and Gardening Services: general repairs, changing light globes and tap washers, building small ramps and installation of smoke detectors and battery replacement, garden maintenance
  • Nutrition and Meal Preparation: assistance with preparing meals at home, assistance with feeding, provision of supplements, dietetics
  • Clinical Care: nursing, allied health and therapy services such as physiotherapy, occupational or physical therapy, podiatry
  • Care Advice: Hume City Council provides quality care advice with a very personal approach to ensure you are receiving the service that you need. Your Care Advisor has a thorough understanding of your local community and will support you to be linked in with services that meet your individual needs.

By choosing Hume City Council as your package provider, you can continue with the services that you may have been receiving through the Commonwealth Home Support Program, with minimal disruption or change and with the staff you know and trust. You can also choose care from a range of other external providers.

What types of packages are available?

There are four levels of Home Care Packages, which are designed to give you the care you need:

Level 1: supports residents with basic care requirements
Level 2: supports residents with low-level care needs
Level 3: supports residents with intermediate care needs
Level 4: supports residents with high-level care needs   

How do I get started?

To receive a Home Care Package, you must be deemed eligible. To find out your eligibility, call My Aged Care on 1800 200 422. The My Aged Care contact centre will take you through the assessment process.

An Aged Care Assessor assessor will determine if you are eligible for a package and which package level best meets your care needs.
