Home and Community Care – Program for Younger People

Home and Community Care – Program for Younger People is funded jointly by the Commonwealth and State Governments. Services that can be accessed at Hume City Council under this program include:

  • Domestic Assistance
  • Personal care
  • Respite care
  • Property maintenance
  • Planned Activity Groups
  • Delivered meals

Eligibility and Assessment

The Home and Community Care – Program for Younger People (HACC-PYP) provides services for younger people with disabilities and their carers. This program applies to people aged up to 65 or up to 50 for Aboriginal and/or Torres Strait Islander person.

Each service has broad eligibility criteria and an assessment process to determine your needs and eligibility. An assessment process helps to ensure everyone receives the care and support they need.

If you need some help at home you will need to meet eligibility requirements for a particular service. These requirements might include a minimum age and a decreased ability to perform basic daily living tasks without some extra help. The assessment will determine your eligibility and identify exactly how much and what type of help you will need.

Do I Need a Referral?

Individuals can refer themselves for our services. Referrals can also come from health service providers, family members and friends.
