Am I Eligible?

Hume's Home and Community Care (HACC) provides services to residents in the following circumstances:

  • those living in a home environment
  • frail and/or aged with little or no able-bodied support
  • residents who have a physical or intellectual disability or a psychiatric illness with little or no able-bodied support
  • carers of people in any of the above groups.

All service users are rated according to our Priority of Access Policy. This ensures that the needs of residents most at risk, and most dependent, are met.

The HACC program is funded jointly by the Commonwealth and State Governments. HACC funds services which are targeted for frail older people, people with disabilities and carers, providing basic support and maintenance to people living at home whose capacity for independent living is at risk.

Do I Need a Referral?

Individuals can refer themselves for our services or from health service providers, family members and friends.

 

Assessment

An assessment is required for all programs to ensure that residents are eligible and suitable for home and community care services. This involves a home visit with one of Council's trained staff. During this home visit, the types and levels of service you need will be negotiated to suit your particular situation. This will form part of a written service agreement.

The service agreement can be reviewed at any time with you and/or your family if there is a change in your circumstances. Following the assessment, a letter will be sent to you confirming the commencement date of the service.

Community Aged Care Packages (CACPs)

Community Aged Care Packages (CACPs) are individually planned and coordinated packages of care tailored to assist older people to remain living independently in their own homes. They are funded by the Commonwealth Department of Health and Aged Care and by client contributions.

Hume Aged and Disability Services manages and coordinates packages of community care services to assist older, frail persons to remain living in their community to eligible clients assessed by the Aged Care Assessment Service.
